Potenzialentwicklung für arbeitslose Menschen zur Neuorientierung (PAN) – ein Projekt zur frühzeitigen Intervention für Menschen mit Rehabilitationsbedarfen durch psychische Beeinträchtigungen
Zusammenfassung:
Der Schwerpunkt des rehapro-Modellvorhabens im Kreis Plön ist die weiterführende Unterstützung von Menschen im Alter von 18 bis 67 Jahren mit psychischen Erkrankungen und/oder drohender Behinderung (i.S. des § 14 Abs. 1 SGB IV), die in der Regel gerade eine Behandlung in einer psychiatrischen Tagesklinik beendet haben. Ziel des Projektes ist es, Menschen eng und möglichst ohne Unterbrechung beim Übergang in eine (Re-)Integration, in Arbeit oder in eine sinnstiftende Tätigkeit zu begleiten, um so eine dauerhafte Stabilisierung der Menschen zu erreichen. Hierdurch soll die nachhaltige gesellschaftliche wie berufliche Teilhabe (wieder-)hergestellt werden. Das Modellprojekt greift innovative Leistungen in einer neuen Maßnahme auf. Der Aufbau des Projektes ist modular und sozialraumgestaltend (Einzelfallcoaching; Gruppenangebote, Arbeitsprojekte und Profiling; Seminarblöcke und Intervisionsgruppen; Jobcoaching/Bürgerliches Engagement).

Abstract:
The focus of the rehapro model project in the district of Plön is the continued support of people between the ages of 18 and 67 with mental illnesses and/or impending disabilities (as defined in § 14 Para. 1 SGB IV), who have usually just completed treatment in a psychiatric day clinic. The aim of the project is to accompany the person closely and without interruption during the transition to (re)integration to work or to a meaningful activity, in order to achieve a lasting stabilisation of the people. The aim is to (re)establish sustainable social and occupational participation. The model project takes up innovative services in a new scheme. The structure of the project is modular and oriented towards the social space (individual case coaching; group offers, work projects and profiling; seminar blocks and intervision groups; job coaching/civic engagement).
